STEP 1: HEALTH HISTORY
Camper health forms are completed online.* The following info is required to complete registration:
The date of your camper’s last tetanus booster
The name and phone number of your camper’s doctor and dentist
Your campers health insurance or Medicaid information
*NOTE: If registering a returning camper from summer 2016, your camper’s health history is complete. You should update changes in health, medicines and doctor's contact info. You will be required to verify accuracy of the information with a drop down "yes" and your name.
*NOTE: IF YOUR CHILD NEEDS SPECIAL MEDICAL ATTENTION, PLEASE CONTACT THE CAMP STAFF BEFORE REGISTERING TO CONFIRM THAT OUR MEDICAL STAFF AND SUMMER STAFF ARE CAPABLE OF PROVIDING THE SAFEST CAMP EXPERIENCE. IT IS OUR DESIRE THAT EVERY CHILD HAVE A CAMP EXPERIENCE; HOWEVER THERE ARE LIMITATIONS TO THE MEDICAL ATTENTION WE CAN PROVIDE. IN THE EVENT A CAMPER HAS ALREADY REGISTERED AND WE ARE NOT ABLE TO MEET THE NEEDS OF YOUR CAMPER AFTER REVIEWING THE HEALTH INFORMATION, WE WILL CONTACT YOU AND WILL REFUND ANY MONEY THAT HAS BEEN PAID.
STEP 2: CABIN MATE REQUESTS
The following conditions must be met to honor any cabin mate request:
Both campers must be registered for the same camp program and age group during the same week
Both campers must request each other (groups of three or more cannot request each other). If a “triangle” has been requested, none of the campers' requests will be honored.
STEP 3: YOUR CHURCH NAME & DISTRICT (if applicable)
During registration you will see a drop-down list of all North Georgia United Methodist churches. The church names are listed by the District in which they are located. Find out which district your church belongs to here: North GA Conference Church Locator. If you are not a member of a United Methodist Church, you will be able to skip this step.
STEP 4: PAYMENT INFO & DEPOSIT, AUTO PAYMENT PLANS, CAMP STORE ACCOUNT
This online system accepts Visa, MasterCard and Discover. You will be required to make the minimum deposit in order to be registered for camp. Until the deposit is made, your camper will not hold a spot during your selected week. If you register after May 1st, full payment is required at the time of registration.

Automatic Payment Plans & Store Accounts
AUTOMATIC PAYMENT PLANS
If you would like to pay through an automatic payment plan, please answer "yes" on the camper profile page during online registration. Once we have received your completed registration, we will set-up your payment plan to charge your credit card over an equal time period until the final payment is due on May 1st.*
*NOTE: All balances are due in full by May 1st.
CAMP STORE ACCOUNTS
"The Miner's Shack," our camp store, is completely cashless during the camp week.* You can deposit money into a store account for your camper during online registration. You may also choose to deposit to your camper's store account at anytime after registration until your camper comes to camp. If you would prefer to mail a check or cash for your camper's store account prior to the camp week, please include your camper's name and the session he or she will be attending. Each camper will go to the camp store with his or her living group more than once during the week.
*NOTE: If campers bring cash to camp, they will not be able to use it in the camp store.
Policies & Release Agreements
After registering, campers may transfer to a different camp session for a $25 fee up until 30 days prior to the opening day of their initial camp session. No transfers may be made within 30 days of the beginning of a session.
The following guidelines apply to refunds:
Notice of cancellation must be received by our Registrar in writing (email or postal mail).
For cancellation more than 30 days before camp, refund will be the amount paid minus the deposit.
There will be no refund for cancellation within 30 days of the camp session.
If camp fees are not paid by May 1st your registration will be canceled and the next available person on the waitlist will be offered the spot.
All fees paid will be forfeited if camper does not check-in on the Sunday of his/her camp session or chooses to go home early.*
Full refunds will only be given when we receive a signed doctor’s note. Doctor's note must be received within 3 days of the cancellation.
*NOTE: No refunds will be made for any reason after the start of the camp
LATE ARRIVAL OR EARLY DEPARTURE POLICY
Our standard 1-week camps start on Sunday and go through Friday; our mini camps go from either Sunday to Tuesday or Wednesday to Friday; and our 10-day camps go from Sunday to the following Tuesday or from Wednesday to the following Friday. Neither late arrivals nor early departures are allowed. Either removing a camper from his or her living group early or adding a camper after the week has begun is disruptive to the group process and to the other eleven campers in the group. If you know that your camper cannot attend the entire week, please select a different camp week.
HEALTH, HISTORY, TRANSPORTATION & USE OF LIKENESS RELEASE
I attest that the health history information I will complete online during the registration process is current, correct and accurately reflects the health status of the camper to whom it pertains. The camper described has permission to participate in all camp activities except as noted by me and/or an examining physician. I give permission to the physician selected by the camp to order x-rays, routine tests, and treatment related to the health of my child for both routine health care and in emergency situations. If I cannot be reached in an emergency, I give my permission to the physician to hospitalize, secure proper treatment for, and order injection, anesthesia, or surgery for this child. I understand the information on this form will be shared on a “need to know” basis with camp staff. I give permission to print or photocopy this form. In addition, the camp has permission to obtain a copy of my child’s health record from providers who treat my child and these providers may talk with the program’s staff about my child’s health status.
I give permission to the camp staff to transport my camper for emergency or programmatic purposes at the discretion of the Director.
For good and valuable consideration, we hereby consent to and authorize the reproduction, publication, and use by North Georgia Camp & Retreat Ministries INC, Glisson Camp & Retreat Center LLC, and their successors and assigns for advertising, commercial, or any other purpose, of any photograph, picture video or likeness of my child or other family members.
