Note: The Staff Update Form will not be accepted without completed Consent Form, Self Disclosure Form, and Electronic Communication and Social Media Form.  These three forms are attached to the staff update form above.

Step 2:  Send in completed Staff Update and Consent Form
Once you have completed the forms, please fax, email or mail them using the information below.  Remember Staff Update forms will not be accepted without completed consent forms.

Important Staff Dates
Staff Update Form Posted online:  September 30th, 2010
Staff Update Forms due:               December 1st, 2010
Staff Agreements mailed:              Mid-December, 2010
Support Staff Training:                  January 14-17, 2010

If you are not offered a staff postion you will be notified as well.  Not all Alumni staff agreements will be mailed at the same time.  Some people will be placed on a waiting list until further notice.  If you have any questions, please email the Program Team or call the front office (706-864-6181).
